Caring by Sharing

Your GP record contains important information about you. Making your record available means that all health workers outside of your GP Practice such as a hospital, walk-in centres, and out of hours services, have the information they need to provide you with the best possible care.
When someone from another health care organisation wants to see your record, they will always ask you for your permission first. If you say ‘no’ they will not be able see any information.
If you do not wish us to share your information in this way, please tell us. Remember that you can change your mind at any stage.
For more information, please speak to a member of staff at the surgery.
